Enrollment form
Completed for every new applicant and for re-enrollment each school year.
Authorised pick-up list
Names and IDs of every adult permitted to collect your child.
Medical and emergency information
Allergies, conditions, medication, and the numbers to call if we cannot reach you.
Field trip consent
Issued per trip, with the destination, schedule, and cost stated.
Photo and media consent
Your written permission for the school to publish photographs of your child. Revocable at any time.
Leave of absence request
For planned extended absence, so work can be prepared in advance.
